To demonstrate whether prolonged prothrombin time is a predictive factor for complicated acute appendicitis in pediatric patients treated at the Virgen de la Puerta Alta Complexity Hospital in the period 2018-2023. Materials and methods: An observational, analytical, case-control study was carried out, the sample was made up of 196 pediatric patients, 98 with complicated acute appendicitis (cases) and 98 patients with uncomplicated acute appendicitis (controls) who met the criteria for selection. Results: The average age of pediatric patients with complicated acute appendicitis was 9 ± 5.8 years, and of those with uncomplicated acute appendicitis was 11.07 ± 5.2 years. On the other hand, 65.3% of patients with complicated appendicitis were male and 34.7% were female. Similarly, the elevated INR was 79.6% in patients with complicated acute appendicitis and 57.1% in patients with uncomplicated acute appendicitis. Leukocytosis occurred in 88.7% of patients with complicated acute appendicitis and in 82.7% in uncomplicated acute appendicitis. At the same time, of the patients who presented complicated acute appendicitis, 71.4% had a prolonged prothrombin time; while only 28.6% had a non-prolonged prothrombin time, compared to those patients who had uncomplicated acute appendicitis in which 46.9% had a prolonged prothrombin time and 53.1% had a non-prolonged prothrombin time. In this sense, prolonged prothrombin time was statistically associated with complicated acute appendicitis, with patients with this analytical finding having a 4.12 times greater risk of having said pathology compared to patients who did not have prolonged prothrombin time. (OR: 4.12, 95% CI 1.76-9.83, p = 0.034). Conclusions: Prolonged prothrombin time is a predictive factor for complicated acute appendicitis
