Abstract: Objetivo: Analizar si el índice de Kirby severo es un factor asociado a mortalidad
en pacientes con neumonía del Hospital II de EsSalud – Cajamarca durante el lapso
de enero 2022 y diciembre 2023.
Metodología: Se realizó un estudio de cohorte prospectiva, en el cual se incluyeron
a 170 pacientes, siendo divididos en 85 pacientes para el grupo de expuesto (con
índice de Kirby severo) y 85 para los no expuestos (con índice de Kirby no severo).
Se consideró la medición de la relación de presión arterial de oxígeno sobre la
fracción inspirada de oxígeno (PaO2/FiO2) al ingreso hospitalario, así como otras
variables como la edad, el sexo, la comorbilidad, comorbilidad pulmonar, la proteína
C-reactiva, leucocitos y necesidad de ventilación mecánica. Se calculó el riesgo
relativo (RR) de cada variable y mediante un análisis multivariado se evaluaron los
factores que de forma independiente se asocien con la mortalidad por neumonía.
Resultados: La incidencia de mortalidad en pacientes con índice de Kirby severo
fue del 75.3%, mostrando diferencias significativas con el grupo que presentó un
índice no severo, siendo la incidencia del 9.4% (p=0.01). Tras el análisis
multivariado, se determinó que el índice de Kirby severo era un factor asociado de
forma independiente con el aumento en la mortalidad por neumonía (RRa: 19.20,
IC 95%: 3.94-93.60, p=0.01). No se observaron asociación con las demás variables
intervinientes en estudio como la edad, el sexo, comorbilidad, comorbilidad
pulmonar, proteína C-reactiva, leucocitos, estancia hospitalaria y ventilación
mecánica (p>0.05 para todos los casos).
Conclusiones: El índice de Kirby severo está asociado con la mortalidad en
pacientes con neumonía
