The purpose of this study was to determine the histopathological changes that occur in dental follicles in relation to pericoronal spaces and position of unerupted third molars extracted from patients attending the Department of Oral and Maxillofacial Surgery Naval Medical Center “Cirujano Mayor Santiago Tavara“. A study correlational and cross-sectional descriptive type was carried out by radiographic and histologic analysis of 128 specimens follicular. Statistical analysis was performed by Chi-square test to assess whether there is statistical association between the variables. Of the participants, 60.9% were female and 39.1% of men aged 15-49 years. A significant percentage of histopathological changes (75.7%) in the group with follicular spaces ≤2.5mm measures was found, when there should have observed pathology, however no statistically significant association between variables was found. Was observed between the presence of histopathological changes and the position of the third molar unerupted according to Pell and Gregory classification, Class IB specifically, there is a statistically significant association (p = 0.031). Radiographic analysis may not be a reliable technique for evaluating follicular disease, since this is not visible in the early stages, be indispensable in histopathological analysis to diagnose the presence of these abnormalities. It is recommended prophylactically extract third molars unerupted, as a measure to prevent the development of high percentages of follicular disease.
