--- Objectives: to determine the existence and magnitude practice of medicine by medical students after the school day, to identify constituents of the medical act and its associated factors. Materials and Methods: A descriptive, observational study quality-quantitative, qualitative phase included interviews with key informants (doctors); phenomenon was delimited generating background; quantitative survey phase, quantitative phase a selfadministered, anonymous survey was conducted; three segments: a) information for the practice of medicine completed the class schedule (existence and constituent elements of the medical act), b) associated factors, c) academic year, and gender. Results: Population: 1024 medical students with tuition 2013 at the Faculty of Medicine of the National University Mayor of San Marcos, 943 were surveyed (92.1%), 686 (72.7%) brought the medicine, the amount increases according to the year of study (1 year 47% the 7th year 92%), the main increase in the 3rd year, constituent elements: assessment (75.2%), prescription (67.6%). Motivators “for help“ (28%), “dedication to service“ (18.2%), student reaction Factors “satisfaction to serve“ (30.5%), “I am capable“ (19%), compensation factors “thank“ (60.7%) and “nothing“ (14%) Factor “money“ only 2.6%. In 257 (27.3%) students who do not practice medicine are factors of motivation “not authorized“ (32.11%) and “is prohibited“ (18.67%). Conclusions: a) Medical students do the practice of medicine after his class schedule, b) Commitment to service and help are variables of this behavior, c) There is no financial gain, d) There is an association between year of study and medical practice, e) increase the prevalence clinical courses, f) The students made their own initiative but not by social demand at the expense of the legal risks. Recommendations: Ethical Decalogue medical student, screenings, tutoring system, awareness of students and teachers, teaching with ethical approach.
