Abstract: La Expansión Rápida del Maxilar (ERM) fue introducida en el siglo XIX y ha sido clásicamente descrita en la literatura como un procedimiento ortopédico indicado en el tratamiento de Deficiencia Transversa Maxilar. Aunque su efecto principal es observado clínicamente en la sutura palatina media y zona maxilar, recientes investigaciones han sugerido que sus efectos ortopédicos alcanzan estructuras esqueléticas profundas del cráneo.
El presente estudio longitudinal tiene como objetivo evaluar cefalométricamente los cambios esqueléticos a nivel de la base craneal en pacientes en crecimiento sometidos a Expansión Rápida del Maxilar. La muestra se constituyó de 10 pacientes (ocho de sexo masculino y dos de sexo femenino), con diagnóstico de deficiencia transversa maxilar que requirieron de Expansión Rápida del Maxilar como parte de su tratamiento ortodóncico en la Universidad Nacional Mayor de San Marcos.
Se obtuvieron radiografías cefalométricas laterales en dos periodos de tiempo, antes del tratamiento (T1) e inmediatamente después de la fase activa de tratamiento (T2). Las variables evaluadas fueron: ángulo NSBa, ángulo NBa/F.H., ángulo SN/ENAPENP, ángulo F.H./ENA-ENP, ángulo SN/F.H., distancia S-N, distancia S-Ba, distancia N-Ba. Los datos fueron analizados estadísticamente usando el test t-Student para muestras pareadas (P < 0.05).
A partir del análisis y discusión de los resultados obtenidos, se concluyó que existen cambios estadísticamente significativos en todas las variables evaluadas post fase activa de expansión. Por lo tanto, se puede especular que la Expansión Rápida del Maxilar no sólo genera efectos esqueléticos locales sino que también produce cambios esqueléticos a nivel de la base craneal.

The rapid maxillary expansion (RME) was introduced in the 19th century and has been classically described in the literature as an orthopedic procedure in the treatment of transverse maxillary deficiency. Although its main effect is observed clinically in the mid palatal suture and maxillary area, recent studies suggest that the orthopedic effects can achieve deep skeletal structures of the skull. The aim of this longitudinal study was to evaluate the skeletal effects of rapid maxillary expansion (RME) on cranial base in growing patients. The sample consisted of ten growing patients (eight male and two female) diagnosed originally with transverse maxillary deficiency, who required rapid maxillary expansion as part of their orthodontic treatment in San Marcos University. Lateral radiographs of each patient were obtained before treatment (T1) and immediately after the active phase of treatment (T2). The evaluated variables were: NSBa angle, NBa/F.H. angle, SN/ENA-ENP angle, F.H./ENA-ENP angle, SN/F.H. angle, S-N distance, S-Ba distance and N-Ba distance. Data was analyzed statistically by using the paired samples Student´s t-test (P < 0.05). According to the statistical analysis, significant changes were found in all the evaluated variables immediately after the active phase of rapid maxillary expansion. Therefore, it can be speculated that RME has had not only local skeletal effects but also produces significant skeletal changes at cranial base level.
