--- Introduction: malnutrition, overweight and obesity before pregnancy constitute higher risk of morbidity and mortality; where weight gain influences unfavorable outcomes related to childbirth and postpartum. In this research one of these results is anemia in the postpartum period. Objective: To determine whether prepregnancy body mass index is associated with anemia in postpartum women treated at the “Instituto Nacional Materno Perinatal” February 2016. Design: observational, analytical - comparative, retrospective and transversal study. Location: Hospitalization B, C and D, of the “Instituto Nacional Materno Perinatal (INMP)”. Participants: The sample consisted of 288 postpartum women distributed in the study group (96 postpartum women with anemia) and comparison group (192 postpartum women without anemia) attending the Hospitalization of “Instituto Nacional Materno Perinatal” in February 2016. Interventions:. The data collection technique was documentary and collection instrument was a data sheet. Chi-square test was used to determine the relationship of prepregnancy body mass index and anemia in the postpartum period; also for the correlation between prepregnancy body mass index and hemoglobin Pearson correlation test was used; both with a significance level of 5%. Results: The frequency of anemia in postpartum women treated at the Hospitalization of “Instituto Nacional Materno Perinatal was 96 cases. Significant relationship between obesity and the diagnosis of anemia in postpartum women was found (p = 0.045). However, the underweight, normal weight and overweight showed no significant relationship (p> 0.05). Most maternal characteristics were similar in both groups, except for the way of termination of delivery, where most of the group with anemia had cesarean delivery (53.1 vs. 41.7%). It was observed that most postpartum women with anemia gained 12 to 17.9 kg. (37.5%) while in whom had no morbidity was a gain of 7 to 11.9 kg (52.1%). It was determined that there is no correlation between the value of hemoglobin and body mass index (p = 0.447). Conclusions: It was determined that the prepregnancy BMI has no significant relationship with anemia in the postpartum period however, it was found that prepregnancy obesity is significantly related to anemia in postpartum women treated at the “Instituto Nacional Materno Perinatal.
