--- This investigation is an experimental, longitudinal and prospective study and had as objective to determine the difference between dexamethasone as preoperative and postoperative therapy on complications following lower third molar surgery. Sample consisted on 60 patients, who attended to the Department of Oral and Maxillofacial Surgery of the Arzobispo Loayza National Hospital, who required lower third molar surgery and met the selection criteria. This patients were randomly distributed in three groups: the first one received 4 mg of dexamethasone preoperatively and 4 mg postoperatively, the second one received 4 mg of dexamethasone only preoperatively and the third one, which received 4 mg of dexamethasone only postoperatively, the administration of the drug for all the cases was intramuscular by gluteal injection. Swelling by distance between facial points and mouth opening by interincisal distance was measured 60 minutes, one day, three days and seven days after surgery, finally, pain by Numeric Scale (NS) was measured 60 minutes, 6 hours, one day, three days and seven days after surgery. Results showed that the effect of the dexamethasone as preoperative therapy is not significantly higher than the effect of the dexamethasone as postoperative therapy on complications following lower third molar surgery.
