In the present study it was evaluated a total of 165 Clinical Histories and x-rays films of patients who had a endodontic treated tooth extracted (Endodontic Failure), attended in the Dental and Oral Surgery Department of the Peruvian Air force Central Hospital, during the period of January to December of 2004; the purpose of this study was to classified the causes of endodontic failures, according to the time after the endodontic treatment and the pulpar condition of the endodontic treated tooth, in four main groups: Endodontic failures, Endodontic failures cause by post Endodontic restoration, Endodontic failures cause by periodontal disease and Endodontic failures cause by Recurrent caries. The results showed 95 Endodontic failures cause by Recurrent caries (57.6%), 45 Endodontic failures cause by post Endodontic restoration (27.3%), 13 Endodontic failures (7.9%) and 12 Endodontic failures cause by periodontal disease (7.3%). According to the Time after the endodontic treatment of the endodontic treated tooth, 19 failures were before 1 year (11.5%), 81 failures were between 1 - 5 years (49.1%) and 65 failures were after 5 years (39.4%), not being a difference statistically significant. According to the pulpar condition of the endodontic treated tooth, 142 failures were Vital (86.1%) and only 23 failures were nonvital (13.9%), being a difference statistically significant. The highest percentage of Endodontic Failures was by Vertical Root Fracture (53.8%), and the highest percentage of Endodontic Failures cause by post Endodontic restoration were by deficient post Endodontic restorations (64.4%), and only 31,3% were Vertical Root Fracture.
