Background: Adolescent pregnancy represents a major problem in public health, since hypertensive disorders such as preeclampsia occur, which is associated with various factors such as early menarche. However, the literature published regarding this association in adolescents in Latin America is scarce. Therefore, the objective of this study was to evaluate whether there is an association between early menarche and preeclampsia in primigravida adolescent pregnant women controlled by demographic, anthropometric and clinical variables. Methods: We carried out a case-control study at the Instituto Nacional Materno Perinatal (INMP) during the years 2014 to 2019 in Peru. A period of six years was covered, so we included a total of 1440 primigravida adolescents distributed in 50 cases and 190 controls for each year, and a conditional logistic regression analysis was used per year. Results: A higher frequency of early menarche was observed in those with preeclampsia (50.7%) compared to patients without preeclampsia (25.4%), with statistical evidence of association (p<0.001). Likewise, the Odds of early menarche in patients with preeclampsia were higher than in those without preeclampsia (OR: 3.02; 95% CI 2.33 - 3.91). Conclusions: Our findings found an association between early menarche and preeclampsia in primigravida adolescent pregnant women in a national reference center. The Odds of early menarche in adolescents with preeclampsia is greater than the Odds of early menarche in those adolescents without preeclampsia.
