In search of contributing to the knowledge of good performance tools that can predict severe COVID-19 disease in a high-altitude environment, our study evaluates the prognostic performance of the APACHE II scale for 28-day mortality in patients with COVID-19 in the Intensive Care Unit at the Ramiro Prialé Prialé National Hospital (level III) in the city of Huancayo located at more than 3000 m.a.s.l. Methodology: Retrospective cohort study in adult patients with severe COVID-19 hospitalized between 2020-2021 in the ICU. Within 24 hours of the patient's admission to the ICU the APACHE II score was calculated and then correlated with a percentage of predicted mortality. Patients were divided into two groups: survivors and deceased, thereby obtaining the actual mortality. The Chi-square test (categorical variables) and the Mann Whitney U test were employed to analyze the difference between medians. A value of p < 0.05 was considered significant and the area under the curve, sensitivity and specificity for the scale were estimated. Results: 325 patients were included; 72.3% were male. The mean age of the patients was 52.8 +/- 11.9 years. The ICU mortality rate was 46.8% (n=152). Multivariate analysis using Cox regression found that leukocyte values (1.00; 95% CI 1.00-1.00), sodium (1.039; 95% CI 1.012-1.066) and age (1.04; 95% CI 1.02-1.06) were favorable for have a higher risk of mortality. The value of the AUC of the APACHE II scale was 0.78 (95% CI 0.07-0.83). A sensitivity of 73.68% and a specificity of 74.57% were also obtained with a cut-off point of thirteen points on the APACHE II scale. Conclusion: The APACHE II score does not present a good prognostic performance for 28-day mortality, given that actual mortality is significantly higher than the prediction of the tool in patients with COVID-19 in an Intensive Care Unit in a high-altitude city.
