Objective: To compare the preferences between children and their parents according to the characteristics of the dental operator and the clinical scenario for dental care. Materials and Methods: An observational, analytical and cross-sectional study was carried out between the years 2022 - 2023. 152 parents and their children, selected by convenience, were surveyed. A validated questionnaire was used and subjected to expert judgment (Aiken's V =0.85), a pilot study was developed (Cronbach's α =0.95) and a kappa =0.86. The bivariate statistical analysis included Fisher's Exact and Chi square tests. Results: Statistically significant differences (p<0.05) were found when comparing the preferences between children and parents for the variables sex, age, clothing, mask, floor, door, lights, curtains, dental chair, play area, waiting area and consulting room. According to the sex of the participants, it was found that the highest percentage of boys (71.76%) and girls (86.57%) prefer to be attended by an operator of the same sex (p=0.001). For clothing; significant differences were found between the children (p=0.001) and according to the characteristics of the clinical scenario (p<0.05) between the parents regarding the preference of curtains and walls; between children for the play area, waiting area and doctor's office; and for both parents and children in the reception area. Conclusion: There are differences when comparing the preferences between children and their parents according to the dental operator and the clinical setting.
