Background: In recent years, a significant increase in internal migration has been seen in Peru, with pregnant women being an important demographic group within this phenomenon. It is estimated that 37% of pregnant women worldwide suffer from anemia, a condition that has repercussions for both maternal and fetal health. Objectives: The aim of the study was to evaluate the association between migration and anemia in pregnant women in Peru during the period 2015-2018. Methodology: The study was cross-sectional and analytical which was based on the Demographic and Family Health Survey (ENDES). The Generalized Linear Model (GLM) with the log Poisson link option was used. Anemia during pregnancy was determined by measuring hemoglobin, adjusted by altitude and classified according to gestational age according to WHO. On the other hand, internal migration was established based on the place where she lived most of the time in her first 12 years of life and the place of current residence, requiring a period of residence greater than 5 years to be considered established migration. Six categories were created to describe the migration flow: (1) Have not migrated, (2) recent migrants (period less than 5 years), (3) urban to urban migration, (4) rural to rural migration, (5) urban to rural migration, (6) rural to urban migration. Results: It was found that 20.92% of pregnant women had anemia. Those who migrated from urban to urban areas in their first and second trimester of pregnancy were twice as likely to have anemia compared to those who did not migrate (PRa: 2.04; 95%CI: 1.04-4.02; p=0.037). Similarly, women who had recently migrated were 70% more likely to be anemic than those who did not migrate (PRa: 1.70; 95%CI: 1.08-2.70; p=0.023). However, in the third trimester of gestation, it was observed that pregnant women who had migrated from urban to rural areas experienced an 88% reduction in the odds of having anemia (PRa: 0.12; 95%CI: 0.03-0.56; p=0.007). Conclusions: This study found that pregnant women who migrate during the first two trimesters of gestation, particularly from one urban area to another, as well as recent migrants, are more associated with anemia. Also, pregnant women in their third trimester of gestation who migrated from an urban to a rural area were less likely to be anemic.
