Introduction: Obesity is a public health problem both nationally and globally. Metabolic surgery is an effective treatment option for this problem, which not only reduces mortality, but also glycemic control, quality of life, and cardiovascular status of patients. Glycosylated hemoglobin has been used as a predictor of complications in various types of surgeries. Its usefulness in the field of bariatric surgery is controversial. Objectives: The objective of the present study is to evaluate the prognostic performance of HbA1C for postoperative complications at 30 days in adults undergoing sleeve gastrectomy in a private clinic in Lima, Peru. Material and Methods: The data was obtained from the database of a Private Clinic in Lima, Peru between 2017 and 2022. 295 patients without a diagnosis of Diabetes Mellitus undergoing sleeve gastrectomy with a record of HbA1c in their medical history were identified. Univariate and bivariate analysis of preoperative data was performed, in addition to a cut-off point for prognostic performance of complications at 30 days using the Youden method. Results: The HbA1c cut-off point was calculated using the Youden and bootstrap methods, resulting in an optimal point of 5.91. This value did not show statistical significance when used as a prognostic predictor (p > 0.05). Conclusion: Glycosylated hemoglobin did not have a good prognostic performance for early complications in patients who underwent sleeve gastrectomy.
