Objective: The overall objective of the study was to determine the association between maternal polycythemia in the third trimester of gestation and low birth weight in newborns attended at Hospital II EsSalud de Cajamarca during the period December 2020 to December 2021. Methods: A secondary analysis of a database from a hospital in Cajamarca, which contains data from a cohort of newborns with low birth weight was performed, which contains data from a retrospective cohort. A total of 944 pregnant women and their newborns were studied during the period December 2020 to December 2021. Hemoglobin concentration in the third trimester of gestation was used as the independent variable, with emphasis on polycythemia (≥14.5 g/dL) and low birth weight as the dependent variable (<2500 g). Multivariate analysis with Poisson regression was performed to address the relationship between maternal polycythemia and low birth weight. RR with 95% CI were calculated. Results: 26.3% of pregnant women had polycythemia in the third trimester of gestation. The mean maternal age was 32 years (RIC 28;36). Low birth weight was observed in 7.1% of the newborns. Hypertensive disease of pregnancy was present in 10.1% of the pregnant women. Polycythemia in the third trimester increased the risk of low birth weight by 60% (RR=1.60; p=0.047; CI=1.01;2.68). For each year of increase in maternal age, the risk of low birth weight increased by 5% (RR=1.05; p=0.02; CI=1.01;1.09). A history of hypertensive disease in pregnancy increased the risk of low birth weight by 3.5 times (RR=3.49; p<0.001; CI=1.99;6.12). Conclusions: Newborns with a history of polycythemia in the third trimester of gestation showed association with low birth weight. It is recommended to conduct studies with a larger sample size and in different high-altitude locations in Peru and around the world.
