Background: The world fertility rate reached 2.5 births per woman until the year 2019, this fertility rate has been conditioned by economic, social, racial, and educational factors of women. In Peru, in 2019, the highest percentage of fertility in rural areas was given by fertile women between the ages of 20 and 24 years old. Objective: The purpose of this investigation is to analyze the inequality in the fertility rate in women of childbearing age in Peru in the years 2009, 2014 and 2019. Method: It is an ecological study. The data was obtained from “Encuesta Demográfica y de Salud Familiar” (ENDES). The Rstudio software was used to quantify the inequality gradient, the inequality slope index and the health concentration index. For this research, the ENDES fertility rate reports for the years 2009, 2014 and 2019 were used as a sample. Result: It was evidenced that the most disadvantaged departments, and with less access to water and electricity, had the highest fertility rates. In addition, with respect to the inequality regression curve, it was observed that the least disadvantaged departments had greater access to family planning and prenatal care. For the year 2019, there is still evidence of an inequality gap in the fertility rate for the variables studied, except for the social determinant of family planning. Conclusion: For the year 2019, there was evidence of a decrease in the inequality gap with respect to the fertility rate in the variables of family income, access to water, access to basic sanitation, prenatal control, access to electricity, literate population and above all in family planning, where it was found that for that year there was no inequality gap.
