Objective: To compare the esthetic perception of the smile according to the variation of the incisal edge of the central incisors and gingival exposure between Orthodontists and Oral Rehabilitation in Lima, Peru. Materials and methods: The study was observational, analytical, and cross-sectional. The evaluation was carried out by a total of 146 specialists in Lima, Peru. A virtual portfolio was prepared with modifications in the incisal edge increments of the central incisors and gingival exposure in two photographs, male and female. These parameters were evaluated using a VAS scale. Likewise, the mean and standard deviation were reported for the variables age and years of experience. In addition, the Mann-Whitney U test and Student's t-test were used for the association of the main study variables. The confidence level was 95% and the significance level was established as p<0.05. Results: Statistically significant difference was found with the female photographs with 1.5mm incisal edge increment with gingival exposure and 1mm, 1.5mm and 2mm increments without gingival exposure. And with the male photographs with 1.5mm incisal edge increment without gingival exposure and 2mm without gingival exposure. Likewise, the female photograph with 2mm incisal edge increments without gingival exposure was preferred by the oral rehabilitators, while for the male sex the photograph with 1.5mm incisal edge increment without gingival exposure was preferred for both specialties. Conclusion: Both specialties agree that gingival exposure detracts from the esthetic perception of the smile. However, there is a slight discrepancy in the esthetic perception of incisal edge increments.
