Objective: To compare the clinical picture in pediatric patients with acute appendicitis at Hospital de Emergencias Pediátricas of Lima, Peru at the time of the SARS-Cov-2 pandemic (2020) in relation to 2019. Material and methods: Retrospective cohort study. The population were pediatric patients (under 18 years of age), admitted to the Pediatric Emergency Hospital with diagnosis of acute appendicitis between the months of March to September of the years 2019 and 2020. The outcomes that were evaluated were preoperative diagnosis, postoperative complications, disease time and hospital stay. Poisson regression was used, from which crude and adjusted RRs with 95% CI were calculated. Results: Children who were admitted in 2020 had, on average, 1,3 times more risk of presenting a disease time greater than 30 hours (p=0,03). No difference was found between the hospital stay of children who attended the hospital for appendicitis between 2019 and 2020, nor in their post-surgical diagnosis. Conclusion: The pandemic influenced children in 2020 to have a higher risk of presenting a longer time of illness, which was reflected in a greater risk of presenting peritonitis or abscess as a pre-surgical diagnosis.
