Introduction: Bariatric surgery was established as the safest and most effective treatment method for people with morbid obesity. In Peru, the most used technique is sleeve gastrectomy. The Obesity surgery mortality risk score (OS-MRS) helps to classify patients at low, intermediate, and high risk for mortality. As for the prediction of morbidity, there are discrepancies about its usefulness. Objectives: Describe the development of complications 30 days after sleeve gastrectomy, according to the Obesity Surgery Mortality Risk Score, in patients from a Peruvian clinic. In addition, determinate the frequency of postoperative complications according to the Clavien-Dindo classification and describe the type of clinical complication according to the Clavien-Dindo classification. Material and methods: Retrospective cohort study. There were 619 patients over 18 years of age with a BMI ≥30kg/m2 undergoing sleeve gastrectomy. Clinical complications were defined as a dichotomous variable and the OS-MRS had the categories: A, B, C. A regression model was initially proposed; however, due to the lack of clinical complications, only univariate and bivariate analysis was performed. Results: 60 patients had clinical complications within 30 days post-sleeve gastrectomy. The prevalence of complications according to the OS-MRS scale was: A (95%), B (5%) and C (0%) (p=0.018). 69.2% of the patients had a IIIb classification according to Clavien-Dindo. Conclusion: Approximately one in 10 patients developed some postoperative complication. The OS-MRS score was not useful for identifying clinical complications at 30 days post-sleeve gastrectomy.
