Background: Breast cancer is the leading neoplasm that causes death in the female population. Early diagnosis and treatment can increase survival time, for which the healthcare provider plays an important role. Taking into account that the majority of Peruvians depend on the public health system, the objective of our study is to evaluate the association between the oncology healthcare provider (private and public) and the treatment delay in women with breast cancer in Lima, Peru. Methods: A retrospective analytical longitudinal study was carried out through the review of medical records in two healthcare providers located in Lima, Peru. A private one, ALIADA Centro Oncológico and a public one, the Daniel Alcides Carrión National Hospital (HNDAC). Treatment delay was tested by the Mann-Whitney-Wilcoxon test and modeled in quantile regressions for the 25th, 50th, and 75th percentiles. Results: The results of the crude and adjusted analysis revealed that the healthcare provider was a significant factor for treatment delay with a time greater than 21 days in the patients with breast cancer treated in the public healthcare center. Among the clinical variables, it was found that having an unknown molecular subtype or histological grade and the indication of chemotherapy as the first treatment increased the delay. Conclusions: The public healthcare provider had a longer treatment delay compared to the private healthcare provider in the management of patients with breast cancer. This is the first report to be carried out in our country and will serve as a basis for future studies that will allow the improvement of the health system.
