Objective: To evaluate the prognostic performance of the PIM 3 score for mortality in children under 18 years of age hospitalized in the Pediatric Intensive Care Unit of a hospital in Lima, Peru. Methods: Retrospective cohort conducted in the Pediatric Intensive Care Unit (PICU) of the Pediatric Emergency Hospital. Patients under 18 years of age admitted to the PICU during the year 2021 were included. Results: One hundred and fifty-one admissions were studied, of which; 52.32% belonged to the male sex, 30.4% belonged to the age group between 12 and 59 months and the median weight was 15 kg. On the other hand, the incidence of mortality for the year 2021 was 7.2%. In the adjusted analysis, the relationship between the PIM-3 score and mortality was maintained (RR=1.57, 95% CI (1.19-2.07), p=0.0001). It was adjusted for the variables: age, sex, weight, patient admission diagnosis, shock diagnosis record, and Glasgow scale. Conclusions: The PIM-3 presented a great prognostic performance for mortality. The patients who died had more than double the score compared to those who lived. So, it is recommended to carry out other studies to evaluate if the basal condition of the patient directly affects the prognostic value of the instrument.
