Background & aims: The neutrophil / lymphocyte ratio has been used as a prognostic factor in various urological cancers such as kidney cancer, bladder cancer, upper tract urothelial carcinoma, and prostate cancer. Prediction of overall survival and progression-free survival outcomes for testicular cancer patients was evaluated with the NLR marker. Methods: A systematic review and meta-analysis were carried out. A priori, all observational studies that had the NLR value as exposure and Progression-free Survival and Overall Survival as outcomes were included; finally, only 6 cohort-type studies were collected. Risk of bias was assessed using the Newcastle-Ottawa-Scale (NOS). Results: The included studies had a total of 1144 participants, corresponding to 6 cohort studies. Conclusion: There is a greater probability of disease progression and mortality when patients have elevated NLR but this turns out not to be statistically significant. Due to the little supporting evidence, NLR in testicular cancer is not recommended in clinical practice.
