Introduction: Primary dysmenorrhea (PD) is the presence of recurrent pain during menstruation in the absence of a disease that can explain the symptoms. This condition affects women in many dimensions and leads to a higher risk of having a low quality of life. It has been suggested that physical activity may decrease the duration and severity of dysmenorrhea; likewise, as a candidate method of prevention or complement in the control of early dysmenorrhea. However, more research is still needed to establish an association between the two. Objectives: This study sought to identify the association between the level of physical activity and primary dysmenorrhea. Methods: An analytical cross-sectional study was conducted at a private university in Peru. The sample n=693 of female students were surveyed online due to the restrictions derived from the COVID-19 pandemic. The IPAQ was used to evaluate “level of physical activity“ and the ENA for “primary dysmenorrhea“. For multivariate analysis, Poisson regression was used. Results: No association was found between level of physical activity and primary dysmenorrhea (p=0.084, IC 95% 0.82 - 1.01). History of a mother with primary dysmenorrhea had an association with a 33% higher risk of PD (p=0.021, IC95% 1.046 -1.712); also, the use of hormonal contraceptives was found to reduce the risk of PD by 10% (p=0.031, IC95% 0.833-0.991). Conclusions: no association was found between level of physical activity and primary dysmenorrhea. Larger-scale studies are recommended.
