Objective: To evaluate the diagnostic performance of the TG/HDL-C index for prediabetes, as well as the determination of cut-off points (according to established sensitivity and specificity) according to gender and BMI in a clinic in Metropolitan Lima. Methods: The present study is of a cross-sectional retrospective analytical type, developed from January 2013 to December 2019 at the Metabolic Medical Institute in Lima, Peru. The participants were all patients over 18 years of age who attended the outpatient services for the first time. Prediabetes was taken as the dependent variable and the TG/HDL-C Index as an independent variable. The area under the curve was calculated using the receiver operating characteristic (ROC) with its respective 95% confidence intervals (CI 95%) and, thanks to the Youden index, the cut-off point for the TG/HDL index was calculated. -C with better sensitivity and specificity, categorized by gender and BMI. Finally, the positive (IEP +) and negative (IEP -) likelihood ratio was calculated to determine the usefulness of the test. Results: The total participants were 797, of which 32,35% and 43,92% of the women and men had prediabetes. For the general population and women, it was shown to be a test of moderate utility (AUC ROC = 0,64 and 0,65), unlike men whose result was an inadequate test (AUC ROC = 0,57). The optimal empirical cut-off points for the general population, women and men, were 2,53; 2,47 and 2,74 respectively. However, when evaluating the sensitivity and specificity, a greater sensitivity was evidenced in men. According to the BMI, in people with normal and overweight BMI, it was observed that it could be a test with a regular useful range (AUC ROC = 0,63). The empirical cut-off points for normal BMI, overweight, and obesity were 1,04; 2,82 and 2,30. However, in the case of normal BMI, this value revealed that the test is useless (p = 0,171). Conclusion: Despite being more sensitive in men, the TG/HDL-C ratio qualifies as a test of moderate utility in the general population and women as a screening method. If there is a value higher than the cut-off points mentioned, it could give us a suspicion of prediabetes, which is confirmed with other additional tests.
