Objective: To synthesize the available scientific evidence regarding the not acute effects of capsaicin on outcomes related to weight control in adults. Methodology: Systematic review of randomized controlled clinical trials published in Spanish or English. The search was carried out in the databases: Medline, EMBASE, CENTRAL and LILACS until June 25, 2021. Additionally, gray literature sources were consulted and the references included in the selected studies were reviewed. The risk of bias assessment was carried out using the Cochrane collaboration tool. Results: Six studies were included. No significant differences were observed between the use of capsaicin or placebo related to changes in body weight in adults. The results in relation to changes in energy metabolism, alteration of fat oxidation and changes in satiety or hunger were not conclusive. The greatest effects of capsaicin were seen with the use of higher doses, given as food, for short periods and when possible confounders were adjusted for. Most of the studies presented low or unclear risk of bias in some domains evaluated. Conclusion: The results of the review did not show a significant or consistent effect of capsaicin on outcomes related to weight control in adults. Studies with a larger sample size, longer follow-up time and that examine in greater depth the effect of different doses and presentations are required to reach more precise conclusions.
