Introduction: Pneumonia is the main cause of infant mortality worldwide. There are few markers used to predict the severity of pneumonia in children. Methods: Retrospective observational cohort study at the Instituto de Salud del Niño San Borja in 2019, this study seeks to identify factors associated with admission to intensive care in children with community-acquired pneumonia. Results: A total of 166 patients diagnosed with pneumonia were evaluated, 94 (56.6%) were male and the median age was 24 months (IQR: 11-48). Most patients had a mild modified PIRO score 136 (81.9%), and 31 (18.7%) patients had complicated pneumonia. 24 (14.5%) patients were admitted to intensive care unit. Age (RR: 0.99, p=0.011, 95% CI=0.98-0.99), severity evaluated with the modified PIRO score (RR: 3.68, p=0.008, 95% CI=1.40-9.68) and the presence of pneumonia complicated (RR: 6.01, p=<0.001, CI 95%=2.59-13.97) were associated with admission to intensive care. Conclusion: Younger patients with pneumonia, with greater severity evaluated with the modified PIRO score and with complicated pneumonia have a higher risk of admission to intensive care unit.
