Introduction: Lead is a toxic metal that affects different systems, organs and tissues. The greatest concern is the adverse effects in children because they are in a stage of growth and development of the nervous system and bone growth. Objectives: Estimate the association between lead level greater than or equal to 5 μg/dl and chronic malnutrition in children aged 1 to 13 years in a district of Callao in 2012 and 2018. Materials and Methods: Observational, analytical cross-sectional study. This is a secondary analysis of the database of the study “Evaluation, control and monitoring the health of the vulnerable population to lead“ conducted by the Regional Health Directorate (DIRESA) Callao. We used data from children aged 1 to 13 years who were surveyed in 2012 and 2018. Blood lead level was categorized into greater than or equal to 5 μg/dL and less than 5 μg/dL. Chronic malnutrition was calculated as height-for-age below two standard deviations (SD), following the WHO guidelines. We used a generalized linear model (GLM) for estimating the prevalence ratio with a significance level of 95%. Results: Out of the 390 children, we found a median age of 6 years and a predominance of males (54.4%). It was estimated that 6.2% of them presented chronic malnutrition and 48.7% of the children had a blood lead level greater than or equal to 5 μg/dL.. Children with a blood lead level greater than or equal to 5 μg/dL are 2.42 times more likely to have chronic malnutrition compared to those with a level less than 5 μg/dL adjusted for sex, anemia, and weight. Conclusions: There is an association between blood lead level greater than or equal to 5 μg/dL and chronic malnutrition. The prevalence of blood lead level greater than or equal to 5 μg/dL was 8.9% and the prevalence of levels less than 5 μg/dL was 3.5%. The main factor related to chronic malnutrition in children of the study was the weight during enrollment.
