Aim: Summarize and analyze the available evidence on the effect of adding fermented dairy products to the treatment of Helicobacter pylori eradication in adults. Method: A systematic review of randomized controlled clinical trials was performed. The search was based on the information found in Medline, Embase, CENTRAL, LILACS and grey literature sources until June 25th, 2021. Meta-analysis was conducted using Review Manager v. 5.4.1. Publication bias was assessed using funnel plot diagrams and Egger's test elaborated in STATA v. 16.0. The risk of bias was evaluated using the Cochrane Collaboration tool. Results: Eight studies were included. Adding fermented dairy products to drug therapy significantly increased the rate of H. pylori eradication (RR: 1.09; 95% CI: 1.03 to 1.15; 1504 participants; I2=9%; p=0.002).This effect was independent of the type and duration of the therapy, and the period of consumption of the fermented dairy product. There were no differences in most safety outcomes, except for the risk of constipation, abdominal pain and headache, which were lower in the groups that consumed fermented dairy. Most studies showed a high risk of performance bias and unclear risk of reporting bias. Conclusion: Adding fermented dairy products improved the eradication rate and reduced the occurrence of some adverse effects associated with H. pylori eradication drug therapy in adults.
