Introduction: Anemia continues to be an unresolved worldwide problem. This study analyzed the inequality gap in the prevalence of anemia in children under 5 years of age, from 2000 to 2015, in the world. An ecological study was conducted at a country and continent level, and the information was obtained from the World Bank's development indicators database. Methods: To explore the relationship between social determinants and the prevalence of anemia in children under 5, measurement methods were used. Thus, it was possible to quantify absolute and relative inequality, the gradient index of inequality and the proportional inequality of the distribution of the prevalence of anemia in children under 5 years of age per continent. Results: Absolute and relative inequality presented a non-significant descending pattern, in all continents. On the other hand, the slope index was negative on all continents. Finally, the 20% of the population with the greatest disadvantage accumulated up to 30% of the prevalence of anemia in children under 5 years of age, while the 20% with the least disadvantage accumulated less than 20% of the disease. Conclusions: Countries with low health spending and lower life expectancy had a higher prevalence of anemia in children under 5 years of age. On the contrary, countries with higher health spending and higher life expectancy had a lower prevalence of anemia. It is important for countries to have evidence on the inequality gap, in order to focus public policies on the most vulnerable population. In addition, it will adequately approach the socioeconomic determinants to generate strategies to prevent and reduce the development of the disease.
