Objective: To determine whether the initiation of preconception ART is a risk factor for adverse neonatal outcomes compared to the initiation of ART during pregnancy in the treatment of HIV in people over 18 years of age at Hospital Belén de Trujillo. Material and methods: Observational, retrospective and analytical study. The sample size was made up of 168 pregnant women with a diagnosis of HIV on treatment with HAART, divided into two groups: 84 pregnant women who started HAART before pregnancy and 84 pregnant women who started HAART during pregnancy. For the analysis, the IBM SPSS Statistics 27 statistical package was used, in addition to the Chi-square test and the calculation of relative risk using 95 confidence intervals and 5% statistical significance. Results: The results showed that marital status (p=0.02), type of ART (p=0.009) and viral load (p=0.000) were significantly associated with the initiation of antiretroviral therapy. The frequency of adverse neonatal outcomes resulting from the initiation of ART before and during pregnancy in HIV treatment was 48.8% and 21.4% respectively. Adverse neonatal outcomes such as low birth weight (p=0.015) and weight for gestational age (p=0.035) were significantly associated with initiation of antiretroviral therapy before pregnancy. Conclusion: Preconception ART initiation is a risk factor for adverse neonatal outcomes compared to ART initiation during pregnancy
