Determine if abdominal computed tomography is effective in the diagnosis of acute appendicitis in patients treated at the “Virgen de la Puerta” High Complexity Hospital during the period 2017 – 2022. Material and methods: This is a retrospective cross-sectional analytical study diagnostic test type. The population was made up of 600 medical records of patients who were admitted to the emergency room for a presumptive diagnosis of acute appendicitis treated at this hospital. The sample was non-probabilistic and consisted of 85 medical records that met the determined selection criteria. Abdominal computed tomography was compared with pathological anatomy (gold standard). The data were organized and analyzed through descriptive and inferential statistics, through the use of frequency tables, contingency tables and bivariate statistical tests. Data on sensitivity, specificity, predictive values, probability ratios and accuracy indices were obtained. Results: The analysis included 85 patients, 43 (51%) were men and 42 (49%) women; The age range was 6 – 80 years with an average age of 35 years. Furthermore, 74% of all patients had an illness time < than 72 hours. Sensitivity values of 96% were found; specificity 86%, positive predictive values 99% and negative 67%, positive probability ratio 6.73 and negative 0.04; The agreement of the abdominal computed tomography with the gold standard test was demonstrated with the accuracy indices = 0.95, Youden index = 0.82 and the Kappa concordance test = 0.724. The computed tomography found that 50% presented phlegmonous appendicitis while the pathological anatomy did so in 32%. The most frequent tomographic findings were changes in the density of periappendiceal fat (46.4%), mural thickening 42.9%, and increase in appendiceal caliber (39.3%). The least frequent were the presence of abscess or plastron (7.1%) and normal appendix (10.6%). Conclusion: It was established that abdominal computed tomography is an effective test for the diagnosis of acute appendicitis.
