To determine the association between the WIfI and Wagner classification systems, with amputation in patients with peripheral arterial disease, at Belen de Trujillo Hospital during the period 2022-2023. Material and method: A retrospective, observational and analytical cohort study was carried out. In which the clinical histories of patients diagnosed with peripheral arterial disease, from the cardiovascular surgery service of the Belen de Trujillo Hospital (HBT) during the period 2022 - 2023, were analyzed. The data collected was analyzed with the IBM SPSS Statistics statistical program and the relative risk was calculated using a bivariate analysis with Pearson's chi-square test and then related to the intervening variables, using a significance level for all tests of 5%. (p < 0.05.) Results: Of 164 patients, 82 patients who were studied with WIfI classification were registered; 41 belonged to the exposed group (grade ≥ 3) of which 35 (85.4%) were amputees; and 41 to the non-exposed group (grade <3) of which 4 (9.8%) were amputees. (RR (95% CI) = 8.75 (3.42 - 22.39), p < 0.001). The other 82 patients were studied with the Wagner classification; 41 belonged to the exposed group (grade ≥ 4) of which 30 (73.2%) were amputees; and 41 to the non-exposed group (grade < 4), of which 14 (34.1%) were amputees. (RR (95% CI) = 2.14 (1.35 - 3.41), p < 0.001). Through the adjusted analysis with Poisson regression, it was found that the WIFI classification (p= 0.045) is associated with amputation when diabetes mellitus (p=0.007) remains constant. On the other hand, in the following adjusted analysis, the Wagner classification variable (p= 0.45) is not associated with amputation, while diabetes mellitus (p=0.001) did show an independent relationship with the result. Conclusion: In our research, the WIFI and Wagner Classification are valid predictors of amputation with a cut-off value ≥ 3 and ≥ 4, respectively. Likewise, we found in the multivariate study diabetes mellitus as a predictor of amputation in patients with peripheral arterial disease.
