To determine whether frailty is a predictor of morbidity and mortality after cholecystectomy in adult patients with acute cholecystitis. Materials and Methods: A nested case-control study was conducted in a cohort at the Hospital Regional Docente de Trujillo during the period January 2019 to December 2022; the sample studied was 86 older adult patients diagnosed with acute cholecystitis who underwent cholecystectomy. Results: Bivariate analysis found that ASA category III score (33.33% vs 5.41%; p = 0.002), hospital stay (11.08 ± 6.14 vs 3.11 ± 1.85 days; p = 0.001) and frailty (75% vs 37.84%, p = 0.016) were associated with the presence of complications. Multivariate analysis to predict complications in patients with acute cholecystitis postoperatively following cholecystectomy in older adults identified ASA score (ORa = 15.26; CI 95% [2.27 - 102.41]) and frailty (ORa = 7.60; CI 95% [1.45 - 39.73]) as independently associated variables. Conclusions: Frailty is a factor associated with the presence of complications and should be considered preoperatively in patients with acute cholecystitis undergoing cholecystectomy.
