Introduction: Varicocele is one of the most important causes of male infertility since it alters the production and quality of semen. Surgical repair is recommended in those patients with altered seminal parameters. Objectives: To determine the rate of improvement of seminal parameters in patients undergoing varicocelectomy by identifying and comparing macroscopic and microscopic alterations before and after surgery. Methods: A literature search was performed in databases such as Pubmed, OVID, EBSCO and Scopus. Selection criteria included longitudinal cohort studies and randomized clinical trials evaluating the rate of improvement of varicocelectomy. Three collaborators independently performed study selection, data extraction, and risk of bias assessment. Relative risks (RR) with 95% confidence interval were calculated for dichotomous data. Continuous data were analyzed considering their arithmetic mean and standard deviation. It was analyzed with a random-effects model using the Mantel-Haenzel method. Results: 513 studies were obtained from which 1 (n=54) was selected. In the study by Anyadike (2016) an improvement rate of 18% (MD 0.18, 95% CI 0.09 to 0.34) post surgical intervention was found. The intervention effectiveness index showed a significant increase in the rate of seminal parameters post-surgery favoring varicocelectomy. A higher rate of improvement in seminal parameters was found in younger patients compared to older adult patients. Conclusion: Varicocelectomy presents a rate of improvement in seminal parameters such as significant increase in ejaculate volume, volume, motility, concentration and morphology of spermatozoa in patients diagnosed with varicocele.
