To identify in patients with acute cholecystitis the degree of severity and preoperative factors associated with complications of laparoscopic cholecystectomy (COLELAP). Material and Methods: The study is an observational, analytical, retrospective case control design. It was performed at Hospital Belén de Trujillo (HBT) in patients who underwent COLELAP for acute cholecystitis. 108 controls and 27 cases. Results: 68.1% of patients were <50 years old, 82.2% were women. Likewise, 68.9% of patients presented grade II severity. Of the pre-surgical factors, the most frequent were: Murphy's sign (86.7%), lithiasis (97.8%) and ALT>1.5 times the normal value (28.9%). Of the four complications studied, intraoperative complications were the most frequent (96.3%), with vascular (48.2%) and biliary (29.6%) lesions being the most frequent. Severity grade II was not associated with complications (p=0.114). Murphy's sign, fever and lithiasis were associated with intraoperative complications. Fever (p=0.0000) and lithiasis (p=0.0060) were associated with conversion to open surgery. Fever was associated with operative time ≥100 min (p=0.0045) and lithiasis was associated with postoperative stay >2 days (p=0.0007). Conclusiones: Patients who underwent COLELAP for acute cholecystitis frequently presented clinical Murphy's sign and lithiasis. Likewise, these two parameters plus fever were associated with intraoperative complications, being these the predominant ones. It is also concluded that grade II was the most frequent, however, it did not show an association with complications, but there is a probability of presenting some complication when these patients with grade II present clinical Murphy's sign, fever and lithiasis.
