To assess whether revascularization with direct mechanical thrombectomy (dMT) is more effective than bridging therapy with intravenous thrombolysis (BT) in achieving functional independence in patients with acute ischemic stroke. METHODS: A systematic review and meta-analysis of selected randomized clinical trials selected on the basis of searches in Pubmed, EMBASE, Cochrane central, Scopus, Web of Science and Lilacs, and gray literature databases, comparing dMT, was performed. It was reported following the PRISMA recommendations. The main outcome was the rate of neurologic functional independence defined as the modified Rankin scale score 0-2 at 90 days. RESULTS: Six randomized clinical trials (2334 patients) with low risk of bias were included in the study. No significant differences were observed for functional independence at 90 days (OR=0.93, 95% CI 0.79-1.09, p=0.37; I2= 0%), and the pooled DR by random effects was -2% (95% CI - 6 to 2%, p=0.36, I2=0%). The lower range of the 95% CI falls within the stricter non-inferiority range of -10% among the included RCTs. Direct mechanical thrombectomy had lower rates of successful reperfusion (OR=0.77, 95% CI 0.62-0.96, p=0.02; I2=0%). No significant differences were found in terms of mean Rankin score, mortality, and symptomatic intracranial hemorrhage between the two groups. CONCLUSION: Our meta-analysis suggests that direct mechanical thrombectomy compared with bridging therapy did not show a greater effect in terms of functional and safety outcomes. Direct thrombectomy is noninferior to bridging therapy based on noninferiority margins.
