To determine if nutritional status is a predictor of 5-year survival in older adults gastrectomized for advanced gastric cancer. Material and Method: The type of research was observational, retrospective, analytical of cases and controls combined with a cohort. There was a sample of 112 older adults, 56 survived after 5 years (case group) and 56 did not survive (control group). The instrument was a collection sheet made up of epidemiological-clinical characteristics, nutritional status and 5-year survival. Data analyzes were carried out by Cox regression, Kaplan Meier curves and the long Rank test calculated in the IBM SPSS-25 program. Results: According to the bivariate analysis, adequate nutritional status (p=0.000), age ≤ 75 years (p=0.017), primary tumor T2 (p=0.001), nodal status N0 and N2 (p=0.000 and p=0.001 respectively), stage I and II (p=0.000 and p=0.007) and primary tumor size <4cm (p=0.035) were significantly associated with 5-year survival. However, the multivariate analysis finally demonstrated that adequate nutritional stage (p=0.008; HR=2.244), age ≤ 75 years (p=0.021; HR=2.239) and stage II (p=0.042; HR=5.144) significantly predict 5-year survival. Furthermore, the survival curves reflected significant differences with a greater probability of survivors in adequate nutritional status (p=0.000). Conclusion: Adequate nutritional status is a predictor of 5-year survival in older adults gastrectomized for advanced gastric cancer.
