To assess whether there is an association between SARS COV 2 infection and prolonged hospital stay in post-operated acute appendicitis patients in the periods from March 2019 to March 2020 and April 2020 to April 2021 MATERIAL AND METHOD: An observational, analytical, retrospective cohort study was carried out, which evaluated 420 post-operated patients for acute appendicitis from the surgery service of the Trujillo Regional Teaching Hospital, of which 336 correspond to the period before the start of the SARS COV2 pandemic. While the remaining 84 correspond to the period during the pandemic. Data were taken from the patients' medical records, calculating the relative risk (RR) and Pearson's chi-square. Statistically significant associations were those in which the p-value was <0.05 RESULTS: The incidence of prolonged hospital stay is 26% in the exposed cohort, while it was only 15% in the unexposed cohort. The RR was 1.76, so the SARS-COV2 coinfection is a risk factor. Regarding the general characteristics of the patients, the most are 29 to 59 years, male, with a time of illness of more than 24 hours, with a pre-surgical time of fewer than 12 hours, proceed from the province of Trujillo, without self-medication, treatment in the daytime watch, and the postoperative diagnosis was of complicated acute appendicitis. CONCLUSIONS: There is an association between SARS COV 2 infection and prolonged hospital stay in postoperative patients with acute appendicitis when a relative risk of 1.76. The variables time of treatment and postoperative diagnosis are associated with prolonged hospital stay according to a p-value <0.05
