Stroke is a significant health problem that is associated with high mortality. Treatment for arterial hypertension with amlodipine may have a protective effect on ischemic stroke due to its independent effects on blood pressure. Objective: To determine if amlodipine is a protective factor for ischemic stroke in patients with arterial hypertension. Material and method: A study was conducted on 117 patients with ischemic cerebrovascular disease (cases) and 468 patients without ischemic cerebrovascular disease (controls), all diagnosed with hypertension and treated at the Victor Lazarte Echegaray hospital between 2016 and 2020. Treatment with amlodipine was considered if the indication was for 6 months or more. The cases were all patients with ischemic cerebrovascular disease, and the controls were randomly selected. Results: 17.9% of patients with ischemic stroke and 14.5% without ischemic stroke received amlodipine as part of their treatment. In the multivariate analysis, the OR of amlodipine as a protective factor for ischemic stroke was 1.14 (95% CI: 0.65-1.99; p=0.632); the intervening variables related to ischemic stroke were male sex with an OR of 2.201 (95% CI: 1.429-3.390, p=0.000) and a history of ischemic heart disease with an OR of 0.218 (95% CI: 0.113-0.423, p=0.000). Conclusion: Amlodipine is not a protective factor for ischemic stroke in patients with arterial hypertension.
