Determine the effect of Colostrum therapy in preterm neonates with very low birth weight on the reduction of the length of hospital stay and the time to reach full enteral feeding. Methods: Systematic review of randomized controlled clinical trials, registered in five databases and selected under inclusion and exclusion criteria. Oropharyngeal colostrum (CO) was compared without CO in very low birth weight preterm infants (≤ 1500 grams and GA ≤ 32 weeks) within 24 hours. The authors independently reviewed the methodological quality of each trial. Results: In total, three studies were included in the systematic review. With 708 participants (OC group: 355; group without OC: 353) the outcomes found were basically secondary; appeared that Colostrotherapy didn’t demonstrate significant differences in the reduction of the time to reach full enteral feeding and the length of hospital stay Conclusions: Our data obtained did not demonstrate significant concordance (between RCTs) with beneficial effects of Colostrotherapy on the duration of time to reach full enteral feeding and the length of hospital stay in preterm infants with very low birth weight. But we emphasize the need for large RCT with an adequate sample size and validated clinical outcomes in preterm neonates
