The objective of the study was determine whether there is a greater risk of adverse reactions to anti-tuberculosis drugs with combined fixed doses compared to individual doses in patients with extrapulmonary tuberculosis. Material and methods: Retrospective cohort study. The sample consisted of 203 patients. The analysis was using the Chi Square test, relative risk (OR) and logistic regression considering significance of 5% and its 95% confidence interval. Results: It was observed that age (p=0.004), sex (p=0.020), HIV infection (p=0.025) and low weight (p=0.008) were associated with adverse reactions. The patients with HIV were in stage IV and 99% of the low weight patients were modified according to their weight. The incidence of adverse reactions to combined fixed-dose anti- tuberculosis drugs was 17.3 per 100 patients. The most affected organs were hepatic (43.90%) and gastric (43.90%), with the moderate severity level (63.41%) being more frequent. The incidence of adverse reactions to anti-tuberculosis drugs with individual doses was 21.2 per 100 patients. Through the multivariate analysis, male sex (p=0.003, RRa= 1.102, CI=0.399-1.806) and low weight (p=0.018, RRa= 1.151, CI: (0.262-2.04) both variables influence a greater probability of risk. of presenting adverse reactions to antituberculous drugs in patients with extrapulmonary tuberculosis. Conclusion: There is no increased risk of adverse reactions to anti-tuberculosis drugs with combined fixed doses compared to single doses in patients with extrapulmonary tuberculosis
