Objective: To determine whether C-reactive protein is a predictor of risk for surgical site infection in patients undergoing open appendectomy. Material and methods: Historical cohort analytical study, which included 188 medical records of open appendectomized patients, of which they were divided into 2 groups: 94 patients with elevated c-reactive protein and 94 with normal c reactive protein at the time of admission for the emergency service. Statistical tests were relative risk (RR) and logistic regression. Results: It is evident that C-reactive protein is significantly associated (p=0.026) and shows a positive predictive value (RR=2.222) for infection at the surgical site. The frequency of surgical site infection in open appendectomy patients was 15.4% (29/188). Furthermore, female sex (p=0.042; OR=3.735) and those who were identified with perforated appendixes (p=0.014; OR=4578) would be risk factors for surgical site infection in open appendectomized patients. Conclusion: C-reactive protein would be a risk predictor for surgical site infection in open appendectomy patients
