Gallstone ileus, is a rare complication of cholelithiasis, characterized by an intestinal obstruction caused by stones that migrate through a cholecystoduodenal fistula formed by recurrent episodes of acute cholecystitis. The incidence is 0.3 -0.5% of complications of vesicular lithiasis, most often in women and older adults. Likewise, cholelithiasis and ethicism are the main causes of higher prevalence for acute pancreatitis and, in a smaller percentage, hypertriglyceridemia. We can obtain the diagnosis of Acute Pancreatitis by showing a minimum of 02 of the following criteria: characteristic abdominal pain, elevated levels of amylase and / or serum lipase three times higher than their normal values or representative imaging findings. We present the case of a 47-year-old woman who comes for billiard colic abdominal pain in the lower hemiabdomen, nausea, persistent emesis, no elimination of stools and scarce flatus, being diagnosed with intestinal obstruction by biliary ileus by the use of an abdominal CT scan, evidencing the Rigler Triad associated with mild acute pancreatitis; appropriate medical treatment was performed and scheduled for exploratory laparotomy, carrying out an ileolithotomy, without post-operative complications, despite its late diagnosis, evolving favorably.
