Objetive: To determine whether age, sex, leukocytosis, nocturnal attention, attention by a resident physician, incomplete anamnesis, abdominal ultrasonography, self-medication and lack of gynecological evaluation are factors associated with the diagnostic error of acute appendicitis. Methods: An observational, analytical, cross-sectional study was carried out, evaluating 208 patients with suspected acute appendicitis who underwent surgery or those who had the other surgical aspect and during the operative act, it was an acute appendicitis, in the Department of Emergencies of the Regional Teaching Hospital of Trujillo during the period February - May 2019. Results: The prevalence of diagnostic error was 16.83%. The analysis of the clinical characteristics of the study showed that age (38.29 ± 21.01 vs 29.86 ± 15.79, p <0.01), the incomplete anamnesis (34.29% vs 19.08%; p <0.05), the white blood cell count (12321.11 ± 4988.94 vs 13935.42 ± 4411.48, p = 0.055) and leukocytosis (51.43% vs 76.88%; p <0, 01) distributed to diagnostic error. The multivariate analysis was identified at age and leukocytosis (leukocyte count> 11000 / uL), is related to diagnostic error and is part of the prediction model; the area under the ROC curve of the model had a diagnostic yield of 71%. Conclusions: Age and leukocytosis (white blood cell count> 11 000 / uL) are predictors of diagnostic error
