The COVID-19 pandemic is probably the biggest challenge we face today, not only because the immediate medical consequences, but also the long-term social and economic impact. Tuberculosis was the main cause of death by infectious agents worldwide for years, until in April 2020 it was overtaken by COVID-19. Peru has 14% of tuberculosis cases in South America, so it has the second place in the ranking. This pathology predominates in the poorest social strata of the country; to face it, the Peruvian government applies some strategies against the disease such as free diagnosis and treatment, however, the results have not been as expected. It is highly probable that, in our country, COVID-19 will have catastrophic effects over the epidemiological situation of tuberculosis, in the midst of a broken health system. The direct impact of the pandemic would affect the provision of medical services, due to limited human and material resources, both for the management of the disease itself and its comorbidities; indirectly, the pandemic threatens to greatly increase the poverty situation. If the estimates proposed are confirmed, we could soon face a major health catastrophe. Tuberculosis, COVID19, the collapse of the health system and conditions of poverty are already on the table to act synergistically and with potentially devastating results on the health of our population, this however can be prevented, if we have the foresight and decision to act now.
