To determine if the surgical Apgar score is associated with the appearance of morbidity and mortality in postoperative laparotomy patients. Materials and methods: The present study is a retrospective cohort, where the participants were patients over 18 years of age, of both sexes, who underwent laparotomy by the emergency and inpatient care service of the Hospital de Alta Complejidad “Virgen de la Puerta”, during January 2020 to December 2023. Results: This study identified that patients undergoing laparotomy who presented a Surgical Apgar score ≤ 4, had a 24 times greater probability of dying, with an RR 24.39 (95% CI 3.49-170.57), while for morbidity there is 5 times greater probability of presenting a major complication during the 30 days after the intervention, with a RR 5.73 (95% CI 2.50-13.10), Surgical Apgar ≤ 4 had a significant association with morbidity and mortality, with a p <0.001. On the other hand, it is observed that the age of the patients who died was an average of 77 years, with an IQR (66-85). Likewise, the associated variables were; age, gender and ASA classification, with a (p 0.002), (p 0.027) and (p 0.000), respectively. Finally, it is observed that the age of the patients with major complications was an average of 73 years, with an IQR (63- 81), the variables that have an association were; age and ASA classification, with a (p 0.032) and (p 0.000), respectively.
