Abstract: Determinar la asociación entre el desarrollo de complicaciones
obstétricas y la infección por SARS-CoV-2.
Metodología: Se realizó un estudio de tipo cohorte retrospectiva con inclusión
de pacientes gestantes en la segunda mitad del embarazo atendidas en el
servicio de Ginecología y Obstetricia del Hospital Distrital Jerusalén entre marzo
de 2020 y agosto de 2022.
Resultados: No hubo diferencias significativas entre la edad y la primigravidez
entre ambos grupos de exposición, en cuanto al control prenatal (CPN) y el
esquema de vacunación COVID-19, hubo predominio de ambos en el grupo de
gestantes con infección por SARS-Cov-2. En cuanto al análisis del riesgo, se
demostró que el infectarse por SARS-CoV-2 incrementa en 1.63 veces la
ocurrencia de complicaciones obstétricas (IC95%: 1.18-2.26, p=0.002). Además,
la infección por SARS-CoV-2 es un factor de riesgo para parto pretérmino (RR:
3.5, IC95%: 1.21-10.16, p=0.012), preeclamsia (RR: 2.11, IC95%: 1.02-4.37,
p=0.019), ruptura prematura de membranas (RR: 2.57, IC95%: 1.14-5.80,
p=0.016) y riesgo de pérdida del bienestar fetal (RR:2.2, IC95%: 1.12-4.33,
p=0.017). Finalmente, tanto el CPN inadecuado y la infección por SARS-CoV-2
fueron incluidos en el modelo de regresión logística resultando ser factores de
riesgo independientes para el desarrollo de complicaciones obstétricas (RRa:
2.223, IC95%: 1.108-4.461 y RRa: 1.589, IC95%: 1.190-2.398,
respectivamente).

Methodology: A retrospective cohort study was carried out with the inclusion of pregnant patients in the second half of pregnancy treated at the Gynecology and Obstetrics service of the Jerusalem District Hospital between March 2020 and August 2022. Results: There were no significant differences between age and primigravity in both exposure groups, regarding prenatal care (PNC) and COVID-19 vaccination status, there was a predominance of both in the group of pregnant women with SARS-CoV-2 infection. The risk analysis showed that becoming infected with SARS-CoV-2 increases the occurrence of obstetric complications by 1.63 times (RR: 1.63 95%CI: 1.18-2.26, p=0.002). In addition, SARS-CoV-2 infection is a risk factor for preterm delivery (RR: 3.5, 95%CI: 1.21-10.16, p=0.012), preeclampsia (RR: 2.11, 95%CI: 1.02-4.37, p= 0.019), premature rupture of membranes (RR: 2.57, 95%CI: 1.14-5.80, p=0.016) and fetal distress (RR: 2.2, 95%CI: 1.12-4.33, p=0.017).
