OBJECTIVE: To identify risk factors associated with antibiotic resistance in patients with diabetic foot infections. MATERIAL AND METHODS: An observational, retrospective, and analytical study was conducted on patients presenting at the Hospital Jose Cayetano Heredia Piura III-1. The study population consisted of patients diagnosed with diabetic foot infection between 2019 and 2021, meeting the established selection criteria. The Chi-square test was employed to determine the relationship between qualitative variables. Additionally, the risk of the proposed factors was associated with their respective 95% confidence interval, according to the statistical method used. RESULTS: The study reveals that in patients with diabetic foot infection, 75.23% were male and 24.77% were female, with statistical significance (p<=0.04). Age, particularly between 35 and 40 years, with an average of 48.29, shows a significant association (p=0.03). The duration of type 2 diabetes (74.31% with 2-5 years) and the presence of diabetic foot for less than 3 months (39.91%) are also associated (p=0.06 and p=0.05, respectively). Biomarkers such as C-reactive protein (>10 mg/L) and glycosylated hemoglobin (>9%) are elevated in 81.65% and 76.61%, respectively, with statistical significance (p<=0.04 and p=0.05). Antibiotic resistance is present in 81.19% of cases, with significant associations for various medications and types of bacteria (p values<=0.04), highlighting the need to consider these factors in clinical management. CONCLUSION: There are risk factors associated with the demographic and clinical profile of antibiotic resistance in patients with diabetic foot infections
