To determine if postpartum depression (PPD) is a risk factor for exclusive breastfeeding failure in mothers of patients at Hospital Belen de Trujillo. Material and methods: A prospective cohort study was carried out on 115 mothers who were discharged from hospital and received an indication for exclusive breastfeeding. The Edinburgh test for postpartum depression was applied in two stages, the first at the time of selection and the second at week 6. Then, a self-efficacy test was performed to identify the group that failed in exclusive breastfeeding. The sample was collected from January to April 2020. In the statistical analysis, the Chi Square test and the Relative Risk were used. Results: It was determined that PPD is a risk factor for exclusive breastfeeding failure (RR=3.09), CI 95% (1.99 - 4.79). The Chi-square test gives as (p): 20.53, indicating a significant p < 0.05. The variables involved in both groups were compared: age, origin, educational level, socioeconomic level and marital status; not finding statistical significance in any of these variables. Conclusion: Postpartum depression is a risk factor for exclusive breastfeeding failure.
