With the aim of determining the effectiveness of oral isotretinoin alone and associated with azithromycin pulses in the clinical remission of moderate to severe acne, protocolizing management schemes according to clinical variables of the population treated in the Dermatology Unit of the Víctor Lazarte Echegaray Hospital, 2020. Material and methods: a prospective cohort observational investigation was developed; where cohort 1 received oral isotretinoin alone and cohort 2 received oral isotretinoin associated with pulses of azithromycin. The research involved 40 patients with acne (n = 40), with different degrees of severity. The study was evaluated and approved by the research committee of the Antenor Orrego Private University. Results: It was found that all the patients of both schemes reached clinical remission of the Acne lesions, with the dose of isotretinoin from 0.5 to 0.6 mg / d both in the monotherapy group and in the combined group. When assessing the side effects of the established treatments, 80% were found with a combined scheme and 65% of the group that only used isotretinoin; being the dryness of skin and mucous membranes the most reported. In the end, all the participants were satisfied with their clinical improvement.The group with both medicament was better than group with monoterapy
