Resumen: Objetivo: Determinar si el índice de choque ajustado por edad (ICAE) es un predictor de mortalidad en pacientes hospitalizados con choque séptico en la unidad de cuidados intensivos (UCI) y emergencia pediátrica del Hospital Belén de Trujillo (HBT) durante el periodo 2017 – 2021.
Material y métodos: Se ejecutó un estudio con un diseño analítico, longitudinal observacional, cohorte, retrospectivo. El cual estuvo compuesto por niños hospitalizadas de uno a catorce años de edad con diagnóstico de choque séptico. Se seleccionaron los valores de frecuencia cardíaca (FC) y presión arterial sistólica (PAS) a las 0, 2, 4, 6 y 24 horas después del diagnóstico de choque séptico para determinar en que momento es más preciso como predictor el ICAE. Se realizó una regresión logística simple para calcular si el ICAE es útil como predictor de mortalidad.
Resultados: Se incluyeron 140 pacientes pediátricos con choque séptico, divididos en 70 niños con índice de choque (IC) normal y 70 niños con IC anormal ambos grupos ajustados por edad. Se obtuvo 64 pacientes sobrevivientes y 6 fallecidos dentro de la población de niños con un índice de choque normal ajustado por edad, mientras hubo 38 pacientes fallecidos y 32 sobrevivientes dentro de grupo con índice de choque anormal ajustado por edad. El riesgo relativo (RR) calculado fue de 6.3 con intervalo de confianza del 95% (2.861 – 14.021). Por último, los valores obtenidos de la relación matemática entre la FC y la PAS a las 0, 2, 4 y 24 horas no mostraron significancia estadística (p > 0,05), mientras que el ICAE a las 6 horas muestra un valor de p igual a 0,000.
Conclusión: Hay mayor cantidad de pacientes fallecidos con IC anormal ajustado por edad que con IC normal ajustado por edad, es decir la elevación del valor del ICAE se asocia con una mayor mortalidad. El ICAE a las 6 horas es más útil para predecir mortalidad
Objective: To determine if the shock index pediatric age-adjusted (SIPA) is a prognostic factor for mortality in patients hospitalized with septic shock in the intensive care unit (ICU) and pediatric emergency at Hospital Belén de Trujillo (HBT) during the period 2017-2021. Material and methods: A study was carried out with an analytical, longitudinal observational, cohort, and retrospective design. Which was made up of hospitalized children from one to fourteen years of age with a diagnosis of septic shock. Heart rate (HR) and systolic blood pressure (SBP) values were selected at 0, 2, 4, 6 and 24 hours after the diagnosis of septic shock to determine when the ICAE is most accurate as a predictor. A simple logistic regression was performed to calculate whether the ICAE is useful as a predictor of mortality. Results: 140 pediatric patients with septic shock were included, divided into 70 children with normal shock index (CI) and 70 children with abnormal CI, both groups adjusted for age. There were 64 surviving patients and 6 deceased within the population of children with a normal shock index adjusted by age, while there were 38 deceased patients and 32 survivors within the group with an abnormal shock index adjusted by age. The calculated relative risk (RR) was 6.3 with a 95% confidence interval (2,861 – 14,021). Finally, the values obtained from the mathematical relationship between HR and SBP at 0, 2, 4 and 24 hours did not show statistical significance (p > 0.05), while the ICAE at 6 hours shows a value of p equal to 0.000. Conclusions: There are a greater number of deceased patients with abnormal age-adjusted CI than with normal age-adjusted CI, that is, an increase in the ICAE value is associated with greater mortality. The ICAE at 6 hours is more useful for predicting mortality
